Request for Change of Emphasis:
A form used to request change of emphasis within the department prior to the registration period.
Request for Renewal of Application:
A form used to renew the application for admission in the following cases:
- Renewal of incomplete application with no admission decision.
- Renewal of accepted application if the validity of the acceptance has passed. (Acceptances are valid for two semesters)
Request to Reactivate Accepted Application:
A form used to reactivate the accepted application for the semester following the one the applicant is accepted for. (Acceptances are valid for two semesters)
Request to Reactivate Returning Students:
A form used to reactivate the file of students who have taken Leave of Absence for one semester or two consecutive semesters.
Readmission Form:
A form used to apply for readmission after withdrawal from the University or after being not enrolled for three consecutive semesters without official withdrawal.
Withdrawal Form:
A form used to withdraw from the University for various reasons, such as: travel, completion of a program or graduation, transferring to another university, etc …
Transcript & Degree Verification Request:
A form used to request the transcript of all university work done, and for the certification of the degrees received from HU as authentic copies.
